c-change logoC-Change for Inclusion is based in Glasgow. C-Change supports Adults with a Learning Difficulty who want to lead an independent life.

It currently supports 36 people living in their own homes and with a lifestyle they choose. C-Change is also developing a number of other services that will help individuals move out of hospital or move on from their current unsustainable living environments.

 
 
diversity matters logoDiversity Matters is based in Scotland but operates more widely. They provide consultancy, facilitation, training and evaluation services. Their aim is to support more inclusive societies and communities.
 
Diversity Matters works with organisations, groups, individuals, families and sometimes with the whole local community including the social care and health system
 
 
elcap logoElcap is a Lothian wide charity supporting people in the region to improve the quality of their lives. ELCAP believes that people should have the support they need, delivered in the way they want, to live their chosen lives.
 
Elcap works in partnership with people and their families, to help them to identify and plan for the kind of life they want to live, and then to support them to achieve this. They also work with local communities to strengthen their capacity to include people with disabilities and other support needs as valued citizens.

is and organisation that for 20 years had been working in Edinburgh and the Lothians. A leader in the ideas and practice of tenant led organisation from the start, Freespace provides round the clock support to a wide range of people in different ways.

 
inclusion glasgow logoInclusion Glasgow is a living example of how one organisation can exist to provide individually tailored, creative, person centred services in a real way which means people with disabilities get a real life. Everyone who uses Inclusion Glasgow's services has an individual plan and budget.
 

 
inspiring inclusion logoInspiring Inclusion wants to inspire, support and challenge you to live and work in ways that make it more likely that the people we care about and support will live rich and full lives and have their contribution welcomed and valued.
 
Inspiring Inclusion does this by providing events, training and consultancy for organisations, communities and individuals.

 Neighbourhood Networks Neighbourhood Networks is a person-centred Scottish charity, launched in 2001 to provide living support networks for people labelled with support needs living in Scotland. It's Living Support Networks have pioneered an innovative and reliable model of supporting people to live in their own homes in their own communities.

Since being launched eleven Networks have been established across Scotland in North Lanarkshire, Argyll and Glasgow.


 
partners for inclusion logoPartners for Inclusion is designed to provide high quality support to individuals with a learning and/or mental health disabilities. It supports people in 4 main areas:
 
People with learning disabilities leaving long stay hospitals; People with mental health problems leaving long stay in patient care; Young people leaving their special schools and looking for jobs, leisure activities etc; People who have lived already in the community but due to lack of support or inappropriate support their service has not worked for them and we have been able to put a tailored service in that supports them well and maintains their life in the community.

sol logoSOL (Support for Ordinary Living) delivers services to men and women with learning disabilities. SOL services are designed to ensure that people are supported to live life to the full within their own homes, families and the community.

Each person has dedicated support or live-in support workers who work in a person centred way to deliver the right amount and type of support required by each individual.

The Thistle Foundation offers individually tailored services that are designed to support disabled people and those with long term health conditions to lead full and active lives. Services are flexible to meet each individual's personal requirements and needs.

 
You Direct is a Support Brokerage service working in the East and West of Scotland it helps people to lead the lives they choose through independent and creative planning and organising of support.
